Job Description & Requirements Title: Physical Therapist – Home Health
Location: Ketchikan, Alaska
Duration: 26 weeks
Shift: Day shift, 5x8 hours (08:00 AM – 04:30 PM), Monday–Friday
(4x10-hour schedule may be available)
Overview & Responsibilities:
Provide skilled physical therapy services to adult and geriatric patients in a home health setting.
\tPerform comprehensive patient evaluations and establish individualized plans of care with measurable short- and long-term goals.
\tDeliver evidence-based physical therapy interventions including therapeutic exercise, gait training, neuromuscular re-education, manual therapy, vestibular interventions, pelvic floor training, and patient/family education.
\tComplete OASIS assessments, reassessments, recertifications, and discharges in compliance with regulatory and professional standards.
\tManage an average caseload of 4 patient visits per day with an average daily census of 26.
\tTreat common diagnoses including COPD, CHF, orthopedic conditions (including joint replacements), neurologic diseases, and cardiopulmonary disorders.
\tMonitor and assess vital signs, pain levels, strength, sensory response, cardiopulmonary status, and overall functional mobility.
\tIdentify environmental barriers in the home and implement safety and injury prevention strategies.
\tCollaborate with interdisciplinary team members including social services and interpretation services.
\tMay float to hospice services within scope if needed.
\tRequires reliable personal transportation for community-based home visits.
\tRequired Experience:
Minimum of 1 year of Physical Therapy experience.
\tMinimum of 1 year of Home Health and/or OASIS Assessment (OA) experience required.
\tFirst-time travelers accepted only if OASIS experience is present.
\tEpic EMR experience preferred but not required.
\tLicensure & Certifications:
Active Alaska Physical Therapist license required (compact license accepted).
\tBLS certification required.
